2. Complete the form by filing in donor information and selecting the method of payment. It is important that we receive this form, as the information is necessary to send you an official receipt.

Accountability
Sunshine Foundation sends to each donor a receipt acknowledging their gift. Receipt(s) can be sent on a monthly basis or once a year. Please specify your preferred method on the donation form.
